Clinical Pharmacy Service at Yale New Haven Hospital

As a visiting scholar , I began to learn clinical pharmacy service at Yale New Haven Hospital (YNHH). Founded in 1826, it is Connecticut’s first hospital and now the fourth largest hospital in America, with1541 beds, 12991 employees, about 250 clinical pharmacists and 250 technicians in Department of Pharmacy.
I studied the clinical pharmacy at YNHH through observing the daily work of clinical pharmacists and completed seven rotations in clinical departments and one rotation named pharmacy administration. Clinical pharmaceutical services included the pharmaceutical care for inpatients and pharmacist clinic for outpatients. The pharmacists provided safe, effective and timely medication therapy for patients, and played an important role in improving the safety and effectiveness of drugs. They did all the clinical work in Epic information system, such as orders/prescriptions verification, dosage adjustment for patients with renal dysfunction, INR monitoring and dosage adjustment of Warfarin, Antimicrobial Stewardship, adverse drug reaction monitoring, medication reconciliation, therapeutic drug monitoring (vancomycin, aminoglycosides, voriconazole, tacrolimus), vaccination, and so on. The content and models of clinical pharmaceutical services were integrated, homogeneous and refined, which was worth being applied in our hospital.
Time flies. My studying ended soon, but have gained a lot of knowledge and experience which broadened my horizon and stimulated some new ideas. I will make unremitting efforts to improve the work of clinical pharmacy.
